Telemonitoring: a Necessity for Chronic Disease Management

Innovative Remote Care for Heart Failure Patients: A.S.Z. and Comarch Partnership
A.S.Z., an innovative public hospital in Flanders, Belgium, offers a wide range of medical services, including general medical care, emergency services, surgery, radiology, pediatrics, cardiology, and oncology. The Aalst campus is designated as a high-specialty site, at the forefront of medical innovation.
In 2021, A.S.Z. made a significant stride in cardiac care by opening a dedicated heart failure clinic. This initiative was led by Dr. Elke De Vuyst a clinical cardiologist with a sub-specialization in heart failure, and Lily Smet, a specialized heart failure nurse with years of experience in cardiology. Their efforts, in close cooperation with the heart rehabilitation department, have laid a strong foundation for comprehensive heart failure management.
Pioneering Telemonitoring in Cardiology
In the end of 2022, the dedicated heart failure clinic at A.S.Z. cardiology department launched a new telemonitoring initiative with Comarch aimed at enhancing the care for patients with chronic heart failure. The collaboration between A.S.Z. and Comarch was established on the ground of a previous partnership with Proximus, who introduced A.S.Z. to Comarch's patient monitoring solutions. Dr. De Vuyst of A.S.Z. highlights the significance of this initiative:
"Telemonitoring allows us to keep a closer eye on our patients' conditions and intervene much earlier when necessary, which is crucial in managing chronic heart failure."
Launching the Remote Medical Care Program
By the spring of 2023, A.S.Z. launched a pilot phase of a remote medical care program designed specifically for patients with chronic heart failure. The program aimed provide continuous remote follow-up over a six-month period for each patient. The primary objectives included improving therapy adherence, enabling self-monitoring, and facilitating just-in-time interventions to prevent future complications and rehospitalizations.
Empowering Patients Through Technology
The implementation of Comarch's e-Care telemonitoring platform and the HomeHealth app was instrumental in this initiative. These tools empowered patients to better manage their condition, increased their awareness and involvement in their therapy, and ultimately reduced the number of complications and hospitalizations. Remote monitoring has also expanded the scope of activities and enhanced the efficiency of ASZ Aalst’s heart failure team.
